What are wrong keywords?
Wrong keywords are those keywords which are not at all relating to the content of the page. I mean on metatag, use only keywords that are related to the page's content.
What is if you already have a domain?
Just go with the domain. I mean that if you are concentrating on SEO for a new site, then it is better to have a keyword rich domain. But now I think that google not considering the domains much. It is now looking to the popularity of the domain. Still keyword rich domain plays big part on SEO.
Underline text which are not links? I hope you do not mean that.
Some SEO expert analysis explains that search engines like Google gives some importance to the tags like b, i, u, ect. because it gives special appearance to the text. Iam not sure about this technique.
6. Named all the images or graphics after your keywords

From this statement, I mean that it is always better to put keywords you targeting on the bigging of the page. Place images after that. Most of site's page begins with image logo. Some search bots index alt tags of images with the text on your page.
It is always better to place your keywords you are targeting on the top rather than on middle or bottom of your page. Great if they are enclosed under header tags.
An example of unnecessary keyword-filling is by putting hidden texts all over your webpage.
Keywords stuffing and hidden text are two different issues.
Many SEO service providers uses this technique to place their sites on the top positions of search results. But I don't think that this technique is now existing, because many search engines now completely ignoring the hidden input tags.
Also don't fill your meta tag, alt image tag, or title tag with keywords. You will get caught and your site will be banned for this!
What should someone add in the meta tags, alt attributes or title tags?
From this statement, I means that if you are concentrating on two keywords: "key1 and key2", then it is not good to put all alt, title, metakeyword tags only with the keywords "key1, key2". Same text on every tag simply makes your site ban from the search engines. Now Google strictly banning many useless sites who tries to fool search bots.
Wasn't all above about on-page optimization?

The main factor which is under offPage optimization is the great backlink, that determines PR for Google. Now search engines like Google largely depends on popularity of the site when they are ordering the search results.
